Below you will see the "MV-9X" form that you use to apply for your Amateur Radio Vehicle License tag.  The instructions say the tag itself is free, but you still pay your advelorum tax.  You will need the Adobe Acrobat Reader to access this.

Attach a copy of your HAM license, and mail form to the address at the top.  Contact the Department of Motor Vehicles for more information.  I have also heard that there isn't a deadline anymore at all, and that it takes about 90 days to get the tag.  

If you don't have the Adobe Acrobat Reader, you can access the older form in jpg for.  This older form has only 5 spaces, and many calls have six characters. These forms were provided by Alex McCarthy, KG4JWW.

Stan Young, WG4RAY, reported on October 13, 2003: "One hint - clean your old tag before going in, and bring a screwdriver -
you'll turn in your old tag when you apply, and have a paper tag until the Amateur Radio tag arrives.  We'll all get new plates next year; the nice folks at the tag office said they are being re-designed."
